Bug lp:#1304657 was caused by confusion around the name Perms. The new name AccessMode should make it clear that its not the literal file permissions but instead the AccessMode passed to open() (i.e. the umask needs to be applied)

Previously some errors would set the Fail flag while some didn't without a clear reason as all errors leave a bad FileFd behind, so we use a helper now to ensure that all errors set the flag.

The code includes quiet a few FIXME's so while all tests are working it shouldn't be used just yet outside of libapt as it might break.
